The Disability Issues and Social support Group
The group were formed because it was felt there was a lack resources for people with a physical or sensory impairment in this area of the county.  We started by having an open day, consulting with disabled people finding out from them what they wanted in their local area.

The Wheelchair Football Group
This project come form service uses that attend the Day Centre. The aim was to develop a wheelchair football group.  We have received two lots of funding to support the group.  My role has been to slowly encourage the group to form a constitution, with the aim of me stepping back. The group now has a constitution and one member in particular has put a lot of work in and has taken over the running and management of the project.

Partner 5 Person Centred Planning case study - Sarah and Mike
Sarah and Mike were receiving support from a Partner 5 homeless project. After six months they decided to do an alcohol detox. “We suggested that the couple would benefit from PCP sessions to help them tackle some of the issues affecting their ability to move on to successful independent living, “ says Sharon, the local Partner 5 PCP facilitator.

NP2 has established an NVQ accredited Peer facilitation course. This course is for service users and has a variety of units including chairing meetings, negotiation, assertiveness and public speaking.

Talk Sport Group
The Group started because people at the Centre liked talking about sport. This perception led to a group being established. It is a friendly group which meets every week at 1.00pm on a Monday to talk about sport.

Non-Member use of the Centre from Partner 2
We currently offer the IT facilities of the Centre to some people who do not meet the criteria for Social Services clients but who find the centre a suitable place to be able to access computers.

The Disabled Tai Chi Chuan Association
The Tai Chi Chuan Group who meet weekly at the Day Centre originally had the Tai Chi tutor funded through centre funds for four years.  The situation changed and the resources were no longer available to fund tutors fees through centre funds.

Jack moved into his own home in June 2006.  In his person centred plan Jack talked a lot about the house he wanted to live in and it is clear that Jack is very proud of his new home.

Partner 5 Person Centred Planning case study - Sally

Sally is a single parent of a seven-year-old daughter. Sally knew she wanted to make changes in her life but she was feeling depressed.  Partner 5 PCP facilitator, Sandy, used PCP tools to help Sally get to know herself and identify her talents.

The Elder Experts Network

As part of the Partnerships for Older People’s Projects (POPPS) funding Partner 4 has established a network of Elder Experts who are willing to be involved in the planning, commissioning and monitoring of service delivery to older people using health and social care services in their city.
